Prescription Refills: 
-Most Prescriptions can be refilled directly through your pharmacy without calling the doctors office. Please phone your pharmacy first.
-If you still need to call us at 303-655-1111 have ready the Medication Name, Medication strength (the number of mg.), and the Pharmacy phone number.  Please include your current phone number.
-Mail order pharmacy prescriptions will be faxed from our office to the mail pharmacy.  Mail orders will always be for a 90 day supply and one year of refills.

Annual Wellness Exams:
Annual wellness exams are a no-copay benefit to you for preventive services.  It is different from the "complete physical" or "annual physical" in past years. The wellness visit includes blood tests, examination, health advice and referral for recommended screeing tests. The wellness visit does not include diagnosing a new illness, treating an old illness, issuing a new presciption.  If we submit charges for an illness the insurance company may decide that it is not wellness and the insurance company may decide that you need to pay for the visit and any lab tests.  We will do our best to make sure that your wellness visit is 100% covered.


Copays and deductibles must be collected at the time of service. 

We expect our patients promptly to pay all charges that we bill to them. If there is a charge from your visit, it means that we have already billed your insurance and the remaining amount is your responsibility. Your insurance company should provide an Explanation of Benefits (EOB) to you regarding charges, payments and your responsibility. If you disagree with the payment or processing of your charges this would be something to discuss with your insurance company.

Payment for our services is due at the time they are provided to you. This includes compayments, coinsurance and deductibles and any charges which have remained unpaid. We will send out statements via mail following your visit. As a courtesy to our patients, we bill your insurance which constitutes an extension of credit.
If no payments or arrangements have been made on your account we will consider turning the account over to our collection Agency, Professional Finance Company. Collections are always a last resort, so please help us to help you maintain your account. Thank you!
